ALL our GP appointments are triaged by a doctor before being booked. That means that rather than calling the practice to make an appointment with a GP, you can simply go online and complete an AccuRx Triage Request. You can request a GP appointment by clicking on the big green banner on the left side of this page or below (if you are viewing our website on a mobile device the green banner may be at the bottom of the page). Patients who are unable to go online to do this themselves will have the option of our reception team completing a request on their behalf. In this way, all patients, whether going online, calling reception, or attending the practice in person, will have their request dealt with in the same way.
There will no longer be any advantage to calling at 8am, or queueing outside the practice before we open. You can contact the surgery at between 8am - 12pm, and have the peace of mind that your request will be reviewed by a GP and the appropriate action will be taken. Your request will now be read by a staff member by the end of the working day, but please be aware that our GPs are reviewing these requests throughout the day, and will review the urgency of your appointment request in order to signpost or book an appointment with the most appropriate member of the healthcare team to help.
Please note this is for GP appointments only; nurse and Healthcare Assistant appointments can still be booked via reception throughout the day.
If our Emergency Triage GP decides they need to see you for a same-day appointment, you should be aware that your appointment may be subject to delay if the GP subsequently has to deal with an emergency whether in the surgery or at a patient's home. In such an event you will be offered an appointment with an alternative GP although there may be a wait to be seen.
In any event we always aim to keep you fully informed.
Crossley Street Surgery offers a range of nurse and Healthcare Assistant appointments, which can be booked in advance. For further information on our clinics and services, please visit our Clinics and Services page. There is no need to ring at 8am to book these appointments. Contact our receptionists at any time between 8am and 6pm.
Nurse and Healthcare Assistant consultations are by appointment ONLY and are available on weekdays between 8.00am and 6.00pm. We are closed on weekends.
To make an appointment with a nurse or a Healthcare Assistant, please telephone (01937) 543 200 or call into the surgery.
Our reception staff will always try to assist with finding a suitable appointment for you, but on occasions we may not have sufficient appointments for a specific nurse or Healthcare Assistant at a suitable time. These appointments can usually be booked up to 4 weeks in advance.
We are constantly trying to improve our appointment system to allow better access to GPs and Nurses.
Appointments with the GP can be carried out face-to-face or by telephone, and you can specify which method you would prefer when booking the appointment with reception. Telephone appointments times are approximate and the GP/Nurse may ring at any time during the morning/afternoon of your appointment.
Extended Access Clinics are now available, including weekend appointments with GPs, Nurses, and Healthcare Assistants. These are held at Wetherby Health Centre on a Saturday morning. Please note this is not a walk-in service and you will need to book an appointment in advance.
The Extended Access service also offers appointments with clinical pharmacists, physiotherapists, and appointments for mental health support.
These are pre-bookable, routine appointments, which may be offered to you by a GP after they have triaged your request for an appointment. For more information please speak to a member of reception.
Home visits are for serious illnesses, and for people who are housebound. Requests for home visits can be made by telephoning the surgery on (01937) 543 200. Please telephone before 10:30am, as this helps the doctors plan their rounds. If you can give the receptionist a brief description of the problem, the doctor will have some idea of the urgency of the call.
In the event of an emergency outside of surgery hours, please ring 111. A trained receptionist will answer your phone call and you will be offered either telephone advice from a doctor, a consultation at the emergency primary care centre where you can request to be seen at either Harrogate or Leeds, a visit by a doctor, or other appropriate treatment. This will be decided on medical need. Further information on the 111 Service may be obtained from a separate leaflet, which is available on request from the practice, or can be downloaded here (there is also an easy-read version available).
If you are unable to keep your appointment please inform the surgery as soon as possible so that your appointment can be used for another patient.
As part of our ongoing efforts to improve patient satisfaction, we have an automated patient check-in, which allows patients to check in without having to queue at Reception. This is located on the right hand side of the Reception area, as you enter the surgery.
The system is simple to use. Just touch the screen and identify yourself using a combination of your gender and date of birth. Your appointment will be confirmed on the screen and you will be directed to the correct waiting area. The GP or nurse will know that you are waiting.
If you have any problems with this system, or if you need to talk to a member of staff, you can, of course, go to Reception, where we will be happy to help you.

If you would like to register for online booking please come into the surgery and speak to a receptionist. In the interests of patient confidentiality, and to protect your privacy, new registrations for SystmOnline must be carried out in person.
If you are already registered for SystmOnline, and would like to have more detailed access to your medical records, or if you are not yet registered for SystmOnline and would like to be, please come into the surgery with a form of photo ID (for example, passport, driving licence, student ID, bus pass, blue badge, photo ID from work etc).

Crossley Street Surgery is a five-partner, semi-rural practice, set in the attractive market town of Wetherby. The practice serves 11,000 patients and is housed in modern purpose-built premises with all the usual facilities for contemporary General Practice, including wheelchair access and a lift. There is a comprehensive Primary Health Care Team and also many other added facilities both NHS and private. The practice is a local provider for joint injections. Private on-site facilities include Physiotherapy. These are all in addition to the regular disease management clinics.
The practice operates under a Personal Medical Services contract with NHS England.
The practice is fully computerised and paperlight accredited. The practice is not a limited partnership.

If you wish to register with our Practice, the easiest and fastest way to do this is using the link below:
- Register with our practice online - this registration form only takes a couple of minutes to complete and will ensure no important information is missed.
You can also visit the surgery in person and ask a member of our reception team for a registration pack. It is vital that you complete all fields on the registration form as any incomplete details may result in a delay in your registration. As soon as you have completed the registration form you will be able to book an appointment with us. If you are registering with the Practice on a permanent basis your records will be automatically sent on from your previous practice.
The NHS Summary Care Record will be used in emergency care. This record will contain information about any medicines you are taking, allergies you suffer from and any bad reactions to medicines you have had to ensure those caring for you have enough information to treat you safely.
Your Summary Care Record will be available to authorised healthcare staff providing your care anywhere in England, but they will ask your permission before they look at it. This means that if you have an accident or become ill, healthcare staff treating you will have immediate access to important information about your health.
Your GP practice is supporting Summary Care Records but as a patient you have the choice to have one or not. If you wish to have one, you do not need to do anything and a Summary Care Record will be created for you. If you do not wish to have one, you need to complete the Opt Out Form and hand it to a member of the GP practice staff with your other registration documents.
Children under 16 will automatically have a Summary Care Record created for them unless their parent or guardian chooses to opt them out. If you are the parent or guardian of a child under 16 and feel that they are old enough to understand, then you should make this information available to them.
You can choose not to have a Summary Care Record and you can change your mind at any time by informing your GP practice.

You have a right to express a preference to receive services from a named clinician in the practice. If you wish to do this please inform the receptionist when you register with the practice or at any time while registered. While we will always endeavour to honour your preference we do however need to make it clear that in the event of an emergency or unavailability we may not be able to do so.

Moving Out of the Area
If you are moving to an address outside of the practice area, you will be asked to register with another GP nearer your new address.
If you move outside of the Leeds PCT area, then you will automatically be removed from the patient list.

In the interests of patient safety, please allow A MINIMUM OF 48 HOURS from the practice receiving your prescription request to collection of your prescription. If your prescription is collected by a pharmacist, please allow 7 days. We are getting an increasing number of requests from patients asking for "urgent" prescriptions to be issued and signed the same day. Please note that you can order your repeat prescriptions up to 10 days prior to their due date.
REPEAT PRESCRIPTIONS
If your doctor agrees, you may obtain medicines by repeat prescription. These may be obtained, giving 48 working hours notice: -
- By completing a prescription request slip or asking the receptionist over the counter, when you are in the surgery;
- By posting the repeat prescription request slip i.e. the right hand side of your last prescription in the prescription box in the foyer or in the normal post;
- By using SystmOnline
There is also a repeat prescription collection service whereby all repeat prescriptions can be sent directly to the local chemist of your choice. Please contact your chosen chemist to arrange this.

Medication Reviews
Medication reviews are GP initiated, based on a thorough review of a patient’s record. Please book a GP medication review appointment only if requested to do so by the practice.
